Use this form to request credit toward your certificate program based on previous post-secondary coursework or recognized industry training (OHS Certificate only).

📌 You may request credit for:
 ➡️ Transfer Credit (completed coursework at a recognized post-secondary institution)
 ➡️ OHS Industry Training Recognition (for up to 21 elective enrichment hours in the OHS Certificate program)

📝 Before you begin:
✔️ You must be admitted to your program
📄 Be ready to submit all required documents (e.g., transcript, course outline, or proof of industry training/designation)
💳 A $125 non-refundable assessment fee must be paid at time of submission
